Follow the steps below to print and sign registration and parent/student contract and return to Miss Vicky during registration hours with first month's tuition.

3. Return completed registration form, signed contract and first month's tuition to Divaz Dance Academy during registration.

4. Print and sign the Recurring Payment Plan

agreement form and return at time of registration.

**There will be a $20.00 Registration fee for each student.

**To register for classes, bring payment, registration form and student/parent contract on the first day of your classes.
